This 18-year-old patient came to us with concerns regarding the appearance and function of her nose. She felt that her nose was severely crooked, deviating noticeably to the left, and she also experienced significant nasal airway obstruction on that side. Additionally, she was displeased with the dorsal hump, drooping tip, and the overall large size of her nose. Her goal was to improve both the function and appearance of her nose through rhinoplasty and septoplasty.
After a thorough evaluation, it was clear that her nasal deviation and airway issues stemmed from a combination of dorsal hump prominence and a severely deviated septum. The plan was to reduce the dorsal hump, straighten and narrow the nasal bones, and correct the deviated septum. Aesthetic enhancements, including refinement of the nasal tip, would also be addressed to ensure the nose harmonized with her facial features, particularly highlighting her eyes.
Surgery took place early in the summer following her freshman year of college. I used an open approach, allowing me to fully visualize and address the components causing the crooked appearance. The first step involved reducing the dorsal hump, followed by fracturing, narrowing, and straightening the nasal bones. Attention was then turned to the septum, which was significantly deviated. A septoplasty was performed to realign the septum into the midline. To support the nasal tip, a septal extension graft was placed. Finally, the nasal tip cartilage was carefully contoured to create an aesthetically pleasing, refined shape.
The patient was called on the evening of surgery and was recovering well. She returned to the office the next day for packing removal and was seen again a week later for suture and splint removal. Tape was applied to the nose for another week to support the healing process. Follow-up appointments were scheduled at one month, six months, and one year.
At her one-year follow-up, the result was remarkable. Her nose was now straight, with the dorsal hump gone, and the refined tip beautifully complemented her facial structure. Her eyes, which the distracting appearance of her nose had previously overshadowed, now became the focal point of her face. Most importantly, she was thrilled with both her appearance and improved nasal function.
